Summary Advocate for and support youth who are homeless and/or street dependent and provide appropriate guidance and unconditional support to help them sustain a more stable lifestyle.
Specific position duties and responsibilities: ·         Develops professional and supportive relationships with youth experiencing homelessness in the Wake County area through the provision of gateway services
·         Plans and initiates street outreach activities in locations where youth experiencing homelessness congregate, to include outreach activities tailored to youth victims of human trafficking or those at high risk for trafficking and exploitation
·         Administers assessments and facilitates the development of goals that support independent living
·         Partners with youth through the provision of case management services and skill building
·         Facilitates youth connections to formal supportive services and access to benefits
·         Plans, teaches and facilitates life skill classes
·         Monitors and evaluates youth’s progress towards identified goals and provides support in addressing barriers as needed
·         Plans and participates in community events and public awareness activities to increase the community’s understanding about the needs of street dependent youth in the community and how to connect youth to the Street Outreach program
·         Shares in the program’s on-call rotation schedule
·         Maintains direct contact with all youth served in accordance with program guidelines
·         Complies with all record management requirements
·         Enters/tracks required data and provides support to the Operations Coordinator and Director in data management efforts
·         Develops and sustains collaborative relationships with community stakeholders that work to address homelessness in the community
Minimum qualifications: ·         Bachelor’s degree in a human service field from an accredited institution. Two years of related experience working with individuals experiencing homelessness.
Knowledge, skills and abilities: Knowledge of local community resourcesExperience with client documentation and file managementStrong written and verbal communication skills
Other requirements: Valid North Carolina Driver’s License and satisfactory driving recordPersonal vehicle for use in conducting home visits and/or transporting clients in the event agency vehicles aren’t availableProof of current/valid car insuranceAcceptable background reportNegative drug screen results
Work environment: Occasional evening and weekend hours required Culture of care and wellness in the office Occasional exposure to outside weather conditions Potential exposure to bloodborne and/or airborne pathogens
